Athletic Clearance Information

Athletic Clearance - Once you create an account, all completed sport paperwork must be uploaded to this site. 

FHSAA Physical Paperwork:
FHSAA EL2 Physical Paperwork - Upload to Athletic Clearance (Make sure all sections are filled out completely. Only page 4 of this form must be uploaded. Common missing items on page 4 – student info at the top, Doctor signature, license number and info, physical completion date and/or parent & student signatures at the bottom.)

NFHS Video Certifications:
NFHS - Must create an account to complete the courses. Courses need to be completed yearly.
Complete video certifications listed below and upload to Athletic Clearance. (Student athlete name must be on certificate.)

• Concussion for Students
• Heat Illness
• Sudden Cardiac Arrest

Healtlh Insurance:
Health insurance card for student athlete needs to be uploaded to Athletic Clearance. If you do not currently hold your own health insurance, you can purchase Student Accident Insurance through the link below.
      School Insurance of Florida - Student Accident Insurance

FHSAA EL3 forms need to be read and electronically signed online by the parent and student athlete on Athlete Clearance.

Statement of consent and Athletic Handbook need to be read and electronically signed on Athletic Clearance.
