HOW DO I REGISTER MY CHILD?

Step 1:   Download and complete the forms below:

Students new to the state of Florida:

1. Birth certificate for all grades (official birth certificate, not hospital certificate)

2. Physical completed by a licensed physician dated no earlier than August 1st of the prior school year

3. Immunization record (Blue Form) *

4. Proof of residency, can be in the form of:
   Rent receipt - Home contract
   Utility bill with parent(s)/guardian(s) name
   Notarized statement from family member with which they reside
   Residency Verification Form

5. Copy of last report card if attended school previously or name an phone number of school previously attended.

Transfers within the State of Florida:
