Remember all additions/changes for health, dental, vision and group life must be completed on the Bentek site. Do not complete and return the election, confirmation & enrollment form. The attachment is for informational purposes only. Also, when viewing the rates you will notice increases to the BOARD CONTRIBUTION, Fl. Blue Dependent Coverage Deductions and a slight increase in FCL Dental Deductions.
IMPORTANT: You must upload dependent verification in the Bentek Portal when adding dependents. If you do not provide dependent verification your request will be denied.
Vendors for ANCILLARY BENEFITS are visiting sites through November 1st to assist with adding/changing ancillary benefits to be effective January 1, 2025. These vendors will also be at the informative meetings.
