Please note, BEFORE a student can try-out or attend practice, both completed forms (EL2 & EL3) and a copy of the insurance card the student is covered under, must be turned into https://athleticclearance.fhsaahome.org/. We are no longer accepting paper copies. Everything needs to be uploaded into the athletic clearence on the FHSAA Home Page.

Any student who arrives to practice without the completed forms and insurance information will be sent home.
